To provide advanced clinical pharmacy services aimed at optimizing the safe and effective use of medications. The Clinical Pharmacist applies evidence-based clinical knowledge to support physicians and healthcare teams in achieving the best possible therapeutic outcomes for patients, particularly in complex or specialized cases.

Responsibilities

• Review and evaluate medication orders for appropriateness, effectiveness, safety, and cost.
• Provide drug information and education to healthcare professionals and patients.
• Monitor patient medication therapy to identify and resolve drug-related problems.
• Participate in multidisciplinary rounds and patient care conferences.
• Develop and implement clinical pharmacy protocols and guidelines.
• Conduct medication use evaluations and quality improvement initiatives.
• Educate and train pharmacy students and residents.
